Rule 111-8-50-.12. Contributions to the Facility

No resident, resident's family, guardian or representative shall be coerced directly or indirectly into contributing to a resident, facility, staff person, or corporation or agency with any financial interest in the facility.

1. (a) Free will contributions may be made for general or restricted purposes. When free will contributions are made by a person for a restricted purpose, such contribution must be used only for the purpose so designated.

2. (b) When a free will contribution is made, a signed receipt shall be issued to the person making the contribution and shall contain the following information:

   1. 1. The name of the person making the contribution;
   2. 2. The date the contribution is made;
   3. 3. The amount and type of the contribution;
   4. 4. The restricted purpose, if any, for which the contribution is intended.
3. (c) The facility shall keep in a central file, copies of all receipts issued in accordance with section (b) of this rule.
