Rule 111-8-28-.02. Application

Any person or persons desiring to establish an eye bank shall apply in writing to the Department for a permit. The application shall include the following information:

1. (a) Name of hospital or medical school;
2. (b) Name and qualifications of medical director of the proposed eye bank;
3. (c) Name of the eye bank facility;
4. (d) Address of the eye bank;
5. (e) Brief description of equipment and facilities;
6. (f) Concise description of scope of service to be offered;
7. (g) Proof of tax-exempt and non-profit status.
