Rule 111-8-16-.06. Records

The facility shall maintain the following records and make them available to authorized Department employees upon request:

1. (a) a copy of the plan and any subsequent changes thereto;
2. (b) records of rehearsals of the plan;
3. (c) records of incidences which required implementation of the plan.
