Each retired member or, if applicable, a joint annuitant, except any person receiving survivor benefits under the teachers' retirement system of the state in accordance with s. 238.07(16), shall be entitled to receive a supplement computed in accordance with s. 112.355 upon:

1. (1) Furnishing to the Department of Management Services evidence from the Social Security Administration setting forth the retired member's social security benefit or certifying the noninsured status of the retired member under the Social Security Act, and
2. (2) Filing written application with the Department of Management Services for such supplement.
