- (1) Any person engaging in the business of issuing donor annuity agreements must submit Form OIR-C1-1208, “Notification to the Office of Insurance Regulation as a Qualifying Issuer of Donor Annuity Agreements Pursuant to section 627.481, F.S.,” incorporated by reference in Rule 69O-136.100, F.A.C., to the Office electronically at floir.gov/iportal on the date on which the person enters into the first of these annuity agreements. The form may be obtained from floir.gov.
- (2) Any person subject to section 627.481, F.S., that fails to submit the required notification form is subject to penalty as provided in section 626.9521, F.S.
