1. (1) The use of an agent by a custodian is prohibited unless specifically authorized by the Chief Financial Officer in writing.

2. (2) To request permission to use a custodial agent, the custodian must submit such request in writing to the Chief Financial Officer and include the following:

   1. (a) proof that the agent meets the definition of a custodian in section 280.02, F.S.;
   2. (b) a complete collateral control agreement, Form DFS-J1-1001, incorporated by reference in Rule 69C-2.004, F.A.C.; and
   3. (c) proof of the agency relationship between the custodian and agent.
