An educational institution that wishes to conduct a program in this state for the prelicensure education of professional or practical nurses shall submit to the department the following:
- (1) A complete program application, Form DH-MQA 1211 (revised 8/18), “Application for New Nursing Program,” which is incorporated herein by reference and may be obtained from the Board office, HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-10373" http://www.flrules.org/Gateway/reference.asp?No=Ref-10373, or at http://floridasnursing.gov/, demonstrating the proposed program meets the requirements of section 464.019, F.S. The application is available from the board office or on the board’s website at http://floridasnursing.gov.
- (2) A review fee of $1,000 for each prelicensure nursing education program to be offered regardless of the location of the instructional site.
