An applicant for licensure by endorsement pursuant to section 456.0145(2), F.S., shall submit his or her application DH-MQA-5103, Revised 8/2025 Mobile Opportunity by Interstate Licensure Endorsement (MOBILE), which is incorporated herein by reference and which may be obtained from http://www.floridahealth.gov/licensing-and-regulation/dietetic-nutrition, the Board office, or at HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18690"http://flrules.org/Gateway/reference.asp?No=Ref-18690. Applicants for licensure shall meet and comply with all requirements in Section 456.0145(2), F.S.
