Pursuant to Sections 120.695 and 456.073(3), F.S., the Board designates the following as minor violations for which a notice of noncompliance may be issued for the first violation thereof:

1. (1) Failure to provide written notice of licensee’s current mailing address and place of practice in violation of Rule 64B7-28.015, F.A.C.
2. (2) Failure to have proof of active insurance available at an establishment as required by subsection 64B7-26.003(4), F.A.C.
3. (3) Failure to include license number or numbers in any advertisement as required by Section 480.0465, F.S.
