   1. (a) Application fee for licensure or certification: $50.00.
   2. (b) Application fee for applicants completing an apprenticeship program: $100.00.
   3. (c) Initial licensure fee: $100.00.
   4. (d) Renewal fee (active or inactive license): $100.00.
   5. (e) Delinquent license fee: $100.00.
   6. (f) Reactivation fee: $100.00.
   7. (g) Change in status fee: $50.00.
   8. (h) Retired status fee: $50.00.

2. (2) Fees applicable to massage establishments:

   1. (a) Application fee: $150.00.
   2. (b) Initial licensure fee: $100.00.
   3. (c) Renewal fee: $100.00.
   4. (d) Change of business application fee: $25.00.
   5. (e) Transfer of location application fee: $125.00.
   6. (f) Delinquent license fee: $75.00.

3. (3) Other fees:

   1. (a) Duplicate license fee: $25.00.
   2. (b) Unlicensed activity fee: $5.00.
