1. (1) Application forms:

   2.    3.    4.    5.    6. (f) All applicants for licensure by endorsement. shall apply to the Department on Form #DH-MQA-5101 (5/2025), “Mobile Opportunity by Interstate Licensure Endorsement (MOBILE)” HYPERLINK "https://flrules.org/Gateway/reference.asp?No=Ref-18611"https://flrules.org/Gateway/reference.asp?No=Ref-18611, which is incorporated by reference herein.
2. (2) Copies of all application forms can be obtained from the Board office at 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257 or from the web at http://www.floridasclinicallabs.gov/licensing. The application must be accompanied by the appropriate application fee required by Rules 64B3-9.001 and 64B3-9.002, F.A.C.
3. (3) After one year from the date of the original submission of an application, a new application and fee shall be required from any applicant who desires to be considered for licensure.
4. (4) With regard to persons who have been issued a social security number by the Federal Government, disclosure of a social security number is mandatory pursuant to Section 456.013(1)(a), F.S., and social security numbers are used to allow efficient screening of applicants and licensees by a Title IV-D child support agency to assure compliance with child support obligations.
