An applicant for licensure by endorsement pursuant to section 456.0145(2), F.S., shall submit as part of his or her application DH-MQA-5101 (Revised 5/2025), “Mobile Opportunity by Interstate Licensure Endorsement (MOBILE)” which is incorporated herein by reference and which may be obtained from HYPERLINK "https://flrules.org/Gateway/reference.asp?No=Ref-18486"https://flrules.org/Gateway/reference.asp?No=Ref-18486, the Board office, or at https://floridasclinicallabs.gov. Applicants for licensure shall meet and comply with all requirements in Section 456.0145(2), F.S.
