- (1) Any person desiring to obtain licensure by endorsement as a speech-language pathologist or audiologist pursuant to section 456.0145(2), F.S., shall apply to the Department of Health and pay the fee required by Rule 64B20-3.0001, F.A.C. The application shall be made on DH-MQA-5101, Mobile Opportunity by Interstate Licensure Endorsement (MOBILE) (Revised 7/2025), hereby incorporated by reference, and may be obtained from HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18602"http://flrules.org/Gateway/reference.asp?No=Ref-18602, or the Board of Speech-Language Pathology and Audiology, Department of Health, 4052 Bald Cypress Way, Bin #C-06, Tallahassee, Florida 32399-3256 or at http://floridasspeechaudiology.gov/resources/.
- (2) The Board shall certify as qualified for licensure by endorsement those applicants who have completed the application form, remitted the nonrefundable application fee and endorsement fee required by Rule 64B20-3.0001, F.A.C., and who have demonstrated that they meet the requirements of section 456.0145(2), F.S.
- (3) In lieu of demonstrating passage of a national licensure examination, the applicant may submit documentation of active certification of clinical competence from the American Speech-Language Hearing Association for licensure as a Speech-Language Pathologist or documentation of active certification from the American Board of Audiology for licensure as an Audiologist.
