- (1) Any person desiring to be licensed as a podiatric physician shall apply to the Board of Podiatric Medicine. The application shall be made on the Podiatric Physician Application for Licensure and Examination Form DH-MQA 1138 (revised 6/2022), hereby adopted and incorporated by reference, that can be obtained from HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-14886" http://www.flrules.org/Gateway/reference.asp?No=Ref-14886, and the web at https://floridaspodiatricmedicine.gov/.
- (2) An applicant for licensure by endorsement pursuant to section 456.0145(2), F.S., shall submit as part of his or her application DH-MQA-5101 (Revised 8/2025), Mobile Opportunity by Interstate Licensure Endorsement (MOBILE), which is incorporated herein by reference and which may be obtained from HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18908"http://flrules.org/Gateway/reference.asp?No=Ref-18908, the Board office, or at the Board’s website https://floridaspodiatricmedicine.gov/. Applicants for licensure shall meet and comply with all requirements in Section 456.0145(2), F.S.
- (3) An application file for licensure is not complete unless and until it contains verification of a passing score from examination of the National Board of Podiatric Medical Examiners, including Part I, Part II, and Part III, as required by Rule 64B18-11.002, F.A.C. Such verification must be received by the Board office directly from the provider of the National Board of Podiatric Medical Examiners examination.
