(1) Requirements for Licensure.
- (a) All applicants for licensure as physician assistants pursuant to Section 458.347, or Section 459.022, F.S., shall apply on Form DH-MQA 2000, entitled “Physician Assistant Application for Licensure,” (revised 12/2021), hereby adopted and incorporated by reference, and can be obtained from the website at HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-14270" http://www.flrules.org/Gateway/reference.asp?No=Ref-14270, or http://flboardofmedicine.gov/licensing/physician-assistant-licensure/.
- (b) All applicants for licensure as physician assistants by endorsement pursuant to section 456.0145(2), F.S., shall submit as part of his or her application DH-MQA-5103 Mobile Opportunity by Interstate Licensure Endorsement (MOBILE), (Revised 8/2025), hereby adopted and incorporated by reference, and can be obtained from the website at HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18738"http://flrules.org/Gateway/reference.asp?No=Ref-18738, or http://flboardofmedicine.gov/licensing/physician-assistant-licensure/.
- (c) Applicants who have been granted a temporary license pursuant to Section 458.347(6)(d) or 459.022(6)(d), F.S., are eligible to seek an extension of the temporary license by submitting Form DH-MQA 1076, entitled “Application for Extension of Temporary Physician Assistant Licensure” (Rev. 06/21), hereby adopted and incorporated by reference, and can be obtained from the website at HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-13887" http://www.flrules.org/Gateway/reference.asp?No=Ref-13887, or HYPERLINK "http://flboardofmedicine.gov/licensing/physician-assistant-licensure/"http://flboardofmedicine.gov/licensing/physician-assistant-licensure/.
- (d) All applicants for licensure as physician assistants for Temporary Certificates for Physician Assistants to Practice in an Area of Critical Need pursuant to Section 458.315, or 459.0076, F.S., shall apply on Form DH-MQA 5088, entitled Temporary Certificate for Physician Assistants to Practice in an Area of Critical Need (Revised 8/2025), hereby adopted and incorporated by reference, and can be obtained from the website at HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18737"http://flrules.org/Gateway/reference.asp?No=Ref-18737, or http://flboardofmedicine.gov/licensing/physician-assistant-licensure/.
- (2) Applicants for licensure pursuant to Section 458.347, or Section 459.022, F.S., who have not passed the National Commission on Certification of Physician Assistants Physician Assistant National Certifying Exam within five (5) attempts and have not practiced as a fully licensed physician assistant shall be required to successfully complete a minimum of three (3) months in a full-time review course at an accredited physician assistant program approved by the Chair of the Council on Physician Assistants. Said completion shall be documented by a letter signed by the head of the program stating that the applicant has satisfactorily completed the course.
- (3) Restrictions. For purposes of carrying out the provisions of Sections 458.347(7) and 459.022(7)(e), F.S., every physician assistant is prohibited from being supervised by any physician whose license to practice osteopathic medicine is on probation.
- (4) Notification to the Department as a dispensing physician assistant shall be made by the supervising physician on the form set forth in subsection 64B15-12.0031(4), F.A.C.
