1. (1) An applicant for licensure by examination or endorsement pursuant to Section 459.0055, F.S., must include a completed application form and appropriate fee as set forth in subsection 64B15-10.002(1), F.A.C. The instructions and application form, DH-MQA 1029, (Revised 5/22), entitled “Osteopathic Physician Application For Licensure” is hereby incorporated by reference, and may be obtained from HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-14609" http://www.flrules.org/Gateway/reference.asp?No=Ref-14609 HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-00961%20" , from the Board of Osteopathic Medicine, 4052 Bald Cypress Way, Bin #C06, Tallahassee, Florida 32399-3256, or from the website at: https://floridasosteopathicmedicine.gov/.
2. (2) An applicant for licensure by endorsement pursuant to section 456.0145(2), F.S., shall submit as part of his or her application DH-MQA-5103 Mobile Opportunity by Interstate Licensure Endorsement (MOBILE) (Revised 8/2025), which is incorporated herein by reference and which may be obtained from HYPERLINK "http://flrules.org/Gateway/reference.asp?No=Ref-18687"http://flrules.org/Gateway/reference.asp?No=Ref-18687, the Board office, or at https://floridasosteopathicmedicine.gov/resources/. Applicants for licensure shall meet and comply with all requirements in Section 456.0145(2), F.S.
3. (3) Such application and fee shall expire one year from the date on which the application is initially received by the Board. After a period of one year a new application and fee must be submitted.
