1. (1) To be eligible to receive reimbursement or loan repayment from the program, applicants in eligible professions must:

   1. (a) Be a United States citizen (U.S. born or naturalized) or a United States national, as defined by 8 U.S.C. §1401 – 1440 (1/3/2022), which is incorporated by reference and available at HYPERLINK "http://www.flrules.org/Gateway/reference.asp?No=Ref-15267" http://www.flrules.org/Gateway/reference.asp?No=Ref-15267,
   2. (b) Possess a clear active Florida health care professional license,
   3. (c) Provide direct patient care to persons in an underserved location in Florida,
   4. (d) Have a qualified loan as defined in subsection 64W-4.001(6) F.A.C.,
   5. (e) Accept Medicaid reimbursement unless the practice is not eligible. The only Medicaid ineligible practices that are eligible under the FRAME program are: correctional institutions; Veteran Affairs clinics; state mental health hospitals; free clinics; public and private school clinics; and state facilities for the developmentally disabled,
   6. (f) Be employed in an a primary care setting as defined in subsection 64W-4.001(11), F.A.C.,
   7. (g) Have a valid email address for program correspondence,
   8. (h) Have not received more than four awards. Awards are not required to be in consecutive years. Awards received prior to the 2024-2025 fiscal year award cycle will not count towards the maximum limit of four awards for each applicant.
   9. (i) Practice in primary care, and

   10. (j) Provide at least 25 hours annually of volunteer primary care services in Florida in a free clinic or through a volunteer program as defined in part IV of chapter 110, F.S. For the purpose of fulfilling this requirement, primary care services include related services which contribute to the provision of primary care (including mental health) at a qualified clinic or program.

       1. Volunteer hours must be completed by the end of the state fiscal year following award acceptance. Individuals may also submit volunteer hours completed between January 1 and June 30 of the current award cycle year if those hours were not used to meet volunteer hour obligations for a prior award.

       2. Volunteer hours will be verified via the FRAME Volunteer Hours Verification Form, incorporated by reference in subsection 64W-4.003(4), F.A.C.

       3. All applicants must complete the FRAME Volunteer Service Obligation Form, incorporated by reference in subsection 64W-4.003(5), F.A.C.

       4. If the applicant does not complete the required 25 hours of volunteer hours in the timeframe specified in subparagraph 1. above:

       a. The applicant will not be eligible in the subsequent calendar year,

       b. The applicant may be subject to the penalties set forth in Rule 64W-4.007, F.A.C., and

       c. After this year of ineligibility, any future application will be considered a new application for the purposes of award prioritization set forth in Rule 64W-4.005, F.A.C.

   11. (k) Does not have any unfulfilled hours associated with an award from the previous application cycle.
2. (2) To be eligible to receive award as an autonomous APRN, autonomous APRN applicants must be actively practicing as an autonomous APRN. Holding an autonomous APRN registration alone does not qualify the APRN for the higher reimbursement or loan repayment amount under Section 381.402(3)(a), F.S.
