The president and a majority of the directors elected at the organization meeting of the incorporators shall make, sign and make oath to a certificate (hereinafter called “articles of organization”) setting forth:

1. (1) A true copy of the articles of association;
2. (2) The names of the subscribers thereto;
3. (3) The name, business, post office or mailing address of each of the officers of the corporation; and
4. (4) The date of the first meeting and the successive adjournments thereof, if any.
