1603 PETITION FORM FOR CANDIDATES AFFILIATED WITH A PARTY NOT ELIGIBLE TO CONDUCT A PRIMARY ELECTION
1603.1 The front of each page of the petition shall contain the name, address and registration number of the candidate; the office to which the candidate seeks election; the ward, school district or single-member district (where applicable); and the political party with which the candidate is affiliated.
1603.2 The back of each page of the petition shall contain the name, address (telephone number optional) of the person circulating the petition and the affidavit of circulator required by this chapter.
1603.3 The back of the petition page shall inform the circulator of the penalties for conviction of providing false information in the affidavit of the circulator required by this chapter.
1603.4 No nominating petition form shall be issued by the Board until the information required pursuant to § 1603.1 has been placed in all appropriate spaces on the petition form.
