511 EMERGENCY MEDICAL TECHNICIAN FEES

511.1 The Department of Health shall collect the following fees for certifying, recertifying, granting reciprocity or extensions, re-testing, or issuing replacement certification cards to emergency medical technicians:

1. (a) Application fee: $15;
2. (b) Initial certification fee: 30;
3. (c) Recertification fee: 30;
4. (d) Reciprocity certification fee: 40;
5. (e) Re-testing fee: 15;
6. (f) Extensions of certification: 10;
7. (g) Replacement cards: 10;
8. (h) Temporary cards: 10.

511.2 The Department of Health shall collect the following fees for certifying, recertifying, granting reciprocity or extensions, or issuing replacement cards to paramedics:

1. (a) Application fee: $25;
2. (b) Initial certification fee: 50;
3. (c) Recertification fee: 30;
4. (d) Reciprocity certification fee: 55;
5. (e) Extensions of certification: 15;
6. (f) Replacement cards: 10;
7. (g) Temporary cards: 10.

511.3 The Department of Health shall collect the following fees for inspecting and licensing ambulances, including inspecting both Basic Life Support ambulances and Advanced Life Support ambulances:

1. (a) Advanced Life Support ambulances: $600;
2. (b) Basic Life Support ambulances: 400;
3. (c) Reinspection of ambulances: 50.

511.4 The Department of Health shall collect the following fees for certifying Emergency Medical Service instructors:

1. (a) Application fee: $5;
2. (b) Initial certification fee: 15;

(c) Recertification fee: 15.

511.5 The Department of Health shall collect the following fees for approving the curriculum of emergency medical services training programs:

(a) Application fee: $100;

(b) Initial certification fee: 100;

(c) Recertification fee: 100.
