4208 REIMBURSEMENT RATES: CASE MANAGEMENT SERVICES

4208.1 Case management services shall be reimbursed at the following levels:

1. (a) For initial assessments, five hundred dollars ($500.00) shall be reimbursed by MAA;
2. (b) For monthly assessments, one hundred and twenty-five dollars ($125.00) shall be reimbursed by the MAA;
3. (c) For annual re-assessments, two hundred and twenty-five dollars ($225.00) shall be reimbursed by the MAA.
