232.1 Each request for a hearing shall be submitted in writing to the Office of Fair Hearings.

232.2 The written request for a hearing shall be submitted personally or by certified mail, return receipt requested. This request may be transmitted through the State Committee of Blind Vendors.

232.3 The request for hearing shall be signed by the complainant or on the complainant's behalf by an authorized representative.

232.4 Each complainant shall request a full evidentiary hearing within fifteen (15) working days after an adverse decision based on an administrative review or, in the absence of an administrative review, within fifteen (15) working days of the occurrence of the action upon which the complaint is based.
