3701.1 A response to a hearing notice shall be in writing and contain the following:

1. (a) An admission or denial of each allegation of fact;
2. (b) A statement as to whether the respondent waives the right to a hearing;
3. (c) A statement of defense, mitigation, or explanation concerning any allegation of fact if any;
4. (d) A request to the Department for a settlement of the proceeding by consent agreement, (if the Department provides this opportunity); and
5. (e) The name and address of the respondent's legal counsel, if any.

3701.2 A request for hearing shall be in writing and contain the following:

1. (a) An admission or denial of each allegation of fact;
2. (b) A statement of defense, mitigation, denial, or explanation concerning each allegation of fact; and
3. (c) The name and address of the requester's legal counsel, if any.
