3517.1 A summary suspension notice shall state:

1. (a) That the license or dealer certification of a food processing operation is immediately suspended and that all operations shall immediately cease;
2. (b) The reasons for summary suspension with reference to the provisions of this Code that are in violation;
3. (c) The name and address of the Department representative to whom a written request for reinspection may be made and who may certify that reasons for the suspension are eliminated; and
4. (d) That the licensee or dealer may request a hearing by submitting a timely request in accordance with section 3700, which request does not stay the Department's imposition of the summary suspension.
