4502.1 A report submitted pursuant to Sections 4506 and 4507 of this chapter shall use the following age categories:

1. (a) Eighteen (18) years of age and younger; and
2. (b) Over (18) years.

4502.2 A report that requires listing a patient's residence under Sections 4506, 4507, and 4511 of this chapter shall use the following categories:

1. (a) District of Columbia;
2. (b) Maryland--Calvert County;
3. (c) Maryland--Charles County;
4. (d) Maryland--Montgomery County;
5. (e) Maryland--Prince George's County;
6. (f) Maryland--St. Mary's County;
7. (g) Maryland--other counties;
8. (h) Virginia--Alexandria City;
9. (i) Virginia--Arlington County;
10. (j) Virginia--Fairfax County (including Fairfax City and Falls Church);
11. (k) Virginia--Loudon County;
12. (l) Virginia--Prince William County (including Manassas and Manassas Park);
13. (m) Virginia--other counties;
14. (n) Other states and foreign countries; and
15. (o) Residence unknown.
