8913 FORMS

8913.1 The Department shall provide all forms for registration of trade name, which shall be used to make the required filings or amendments and which will be available from the following:

(a) Business License Center of the Department; and
(b) Other distribution points, including electronic medium, as the Director deems appropriate.
