8911 FEES AND REFUNDS

8911.1 The Department shall charge and collect the following fees:

(a) $ 50.00 for registration of each trade name;

(b) $ 50.00 for renewal of each trade name;

(c) $ 25.00 for each amendment to a registered trade name;

(d) $ 25.00 for each cancellation of a registered trade name;

(e) $ 10.00 for each Certificate of Fact, Certificate of Good Standing;

(f) $ 25.00 for each Certified copy of Certificate of Fact;

8911.2 An administrative fee of up to seven (7) percent shall apply to each fee transaction made through the use of an electronic medium, including the Internet.

8911.3 No refund shall be made of fees paid unless registrant can verify in writing an overcharge to the Director. This verification shall include proof of payment.
