Rule 3 – Requirement for Reinstatement The purpose of this rule is to state the requirements for reinstatement of a license or registration that lapsed due to nonpayment of the annual renewal fee under § 12-29.5-104(4), C.R.S.

A. An acupuncturist may reinstate a license or registration that has lapsed due to nonpayment of the annual fee within 2 years provided that the acupuncturist has met the following requirements:

1. 1. Paid the appropriate renewal fee in effect at the time of reinstatement;
2. 2. Submitted a completed renewal questionnaire; and 3. Provided proof of malpractice insurance in the amount required by statute.
