An entity is eligible for direct payment of an authorized fee if the entity:

1. (a) Has an Employer Identification Number;
2. (b) Has registered with us in the manner we prescribe;
3. (c) Has not been found ineligible for direct payment;
4. (d) Designates and maintains an employee who is registered as a representative in the manner we prescribe as a point of contact to speak and act on the entity's behalf;
5. (e) Accepts payment via electronic funds transfer; and
6. (f) Conforms to our rules.
