(a) Receipts for Receiving Money for Transmission.
- (1) Provide a sample form of receipt for transactions that involve money received for transmission.
- (2) Provide a certification by an officer of the applicant which certifies that the sample form of receipt is the receipt form used by the applicant and its agents.
- (3) Provide a translator's certificate, if applicable, in accordance with Section 5.3002, Title 10, California Code of Regulations.
(b) Payment Instruments.
- (1) Provide a specimen copy of each form of payment instrument issued by the applicant which the applicant proposes to sell in the state of California.
- (2) In case the applicant proposes to sell a form of payment instrument only in certain circumstances (for example, only through a certain California agent or type of California agent or only in certain geographical areas), describe such circumstances.
