1. (a) In case the applicant proposes to have any California agents, provide the following information:

   1. (1) The name and address of each proposed California agent, if known.
   2. (2) Describe the types of business conducted by the applicant's proposed California agents.
   3. (3) Provide a copy of each form of contract which the applicant proposes to make with its California agents in accordance with Financial Code Section 2060.

2. (b) If the applicant has or proposes to have agents outside of California, provide the following information:

   1. (1) The current number of agent branch offices located outside of California.
   2. (2) The estimated number of agent branch offices located outside of California as of the end of one year.
