1. (a) In case the applicant is a California corporation or limited liability company, provide a Certificate of Good Standing issued by the Secretary of State of the State of California as of a date within 30 days before the filing of the application, certifying that the applicant is a corporation or limited liability company which is duly organized, subsisting, and in good standing under the laws of the State of California.
2. (b) Provide a description of the structure or organization of the applicant, including any parent or subsidiary of the applicant, and whether any parent or subsidiary is publicly traded.
3. (c) In case the applicant is a corporation, provide a copy of the Articles of Incorporation. In case the applicant is a limited liability company, provide a copy of the Articles of Organization and operating agreement. If the operating agreement is oral, provide a summary signed by all of the members of the matters contained in the operating agreement.
