Information Regarding Corporate or Limited Liability Matters.

- (a) In case the applicant is a California corporation or limited liability company, provide a Certificate of Good Standing issued by the Secretary of State of the State of California as of a date within 30 days before the filing of the application, certifying that the applicant is a corporation or limited liability company which is duly organized, subsisting, and in good standing under the laws of the State of California.
- (b) Provide a description of the structure or organization of the applicant, including any parent or subsidiary of the applicant, and whether any parent or subsidiary is publicly traded.
- (c) In case the applicant is a corporation, provide a copy of the Articles of Incorporation. In case the applicant is a limited liability company, provide a copy of the Articles of Organization and operating agreement. If the operating agreement is oral, provide a summary signed by all of the members of the matters contained in the operating agreement.
