1. A. Under A.R.S. § 17-332(C), the Department and its license dealers may issue a duplicate license, tag, or electronic tag to an applicant who:

   1. 1. Pays the applicable fee prescribed under R12-4-102, and
   2. 2. Signs an affidavit. The affidavit is furnished by the Department and is available at any Department office or license dealer.

2. B. The applicant shall provide the following information on the affidavit:

   1. 1. The applicant’s personal information:

      1. a. Name;
      2. b. Department identification number, when applicable;
      3. c. Residency status and number of years of residency immediately preceding application, when applicable;

   2. 2. The original license or tag information:

      1. a. Type of license, tag, or electronic tag;
      2. b. Place of purchase;
      3. c. Purchase date, when available; and

   3. 3. Disposition of the original tag for which a duplicate is being purchased:

      1. a. The tag was not used and is lost, destroyed, mutilated, or otherwise unusable; or
      2. b. The tag was attached to a harvested animal that was subsequently condemned and the carcass and all parts of the animal were surrendered to a Department employee as required under R12-4-112(B) and (C). An applicant applying for a duplicate tag under this subsection shall also submit the condemned meat duplicate tag authorization form issued by the Department.
3. C. In the event the Department is unable to verify the expiration date of the original license, the duplicate license shall expire on December 31 of the current year.
