1. A. The Department shall waive the initial license fee when an eligible applicant as identified under subsection (B) requests an initial license for any license listed under subsection (C). At the time of application, the eligible applicant shall submit to the Department the applicable license application and a signed licensing fee waiver form affirming the information provided on the form is true and accurate. The license application and licensing fee waiver forms are available from any Department office and on the Department’s website. The applicant shall provide all of the following information:

   1. 1. Type of exemption, see subsection (B); and

   2. 2. Applicant’s:

      1. a. Name;
      2. b. Date of birth;
      3. c. Mailing address;
      4. d. Email, if available;
      5. e. Telephone number;
      6. f. Customer ID number;
      7. g. Affirmation that the information provided on the application is true and accurate; and
      8. h. Signature and date.

2. B. Under A.R.S. § 41-1080.01, persons eligible for the initial license fee waiver are limited to any:

   1. 1. Individual whose family income does not exceed 200% of the current federal poverty guidelines,
   2. 2. Active military service member’s spouse, or
   3. 3. Honorably discharged veteran who has been discharged not more than two years before application.

3. C. The Department has determined the following licenses may be used for the purpose of operating a business or providing a service in Arizona and are subject to A.R.S. § 41-1080.01:

   1. 1. Aquatic Wildlife Stocking License,
   2. 2. Fur Dealer’s License,
   3. 3. Game Bird Field Training License,
   4. 4. Game Bird Field Trial License,
   5. 5. Game Bird Shooting Preserve License,
   6. 6. Guide License,
   7. 7. Live Bait Dealer’s License,
   8. 8. Private Game Farm License,
   9. 9. Sport Falconry License,
   10. 10. License Dealer’s License,
   11. 11. Taxidermist License,
   12. 12. Trapping License,
   13. 13. Wildlife Holding License,
   14. 14. Wildlife Service, and
   15. 15. Zoo License.
4. D. An applicant for a license fee waiver shall certify they meet the eligibility criteria proscribed in subsection (B), as applicable.
5. E. All information and documentation provided by the applicant is subject to Department verification.
