An outline of coverage to a prospective applicant for long-term care insurance must follow a standard format and content for an outline of coverage, subject to the following standards:

1. (1) the outline of coverage must be a free-standing document, using no smaller than 10-point type;
2. (2) the outline of coverage may not contain advertising material;
3. (3) text that is capitalized or underscored in the standard format outline of coverage may be emphasized by other means that provide prominence equivalent to the capitalization or underscoring;
4. (4) use of the text and sequence of text of the standard format outline of coverage is mandatory, unless otherwise specifically indicated; and
5. (5) the outline of coverage must follow the format as prescribed by the director.
