Wis. Admin. Code § UWS 23.05 – Application process.

(1) Applications shall be in such form as the Board of Regents may require and shall contain the following information:
1. (a) Evidence pertaining to dates of accredited medical or dental school attendance and graduation.
2. (b) Evidence of educational loan indebtedness and education loan outstanding balance.
3. (c) The location of the applicant’s current or proposed practice.
4. (d) Any other information the Board of Regents may require to make a determination in accordance with the provisions of s. 36.60, Stats., and this chapter.
(2) The Board of Regents shall provide application materials.
