Wis. Admin. Code § TCS 4.10 – Notice of hearing.

(1) Form. Notice of hearing shall be in writing, with a title identifying the matter to be set for hearing.
(2) Content. The notice of hearing shall contain the following information:
1. (a) Date, time and location of hearing;
2. (b) The reason for the hearing;
3. (c) A short summary of the matter to be considered;
4. (d) Any other information as the board may deem appropriate.
