A chart order shall contain all of the following:

1. (1) First and last name of the patient.
2. (2) Patient’s medical record number or date of birth.
3. (3) Date of issuance.
4. (4) Name, strength, and form of the drug product or device prescribed.
5. (5) Directions for use.

6. (6) The signature by one of the following methods:

   1. (a) If handwritten, the practitioner’s or delegate’s signature.
   2. (b) Electronic signature of the practitioner or delegate.
7. (7) Chart orders prepared by a delegate of the practitioner shall include the first and last name of the delegate and the first and last name of the practitioner.
