1. (1) The definitions in s. 635.02, Stats., apply to this subchapter.

2. (2) In this subchapter, “health care provider” means any of the following:

   1. (a) A medical or osteopathic physician, podiatrist, physical therapist or physician’s assistant licensed or certified under ch. 448, Stats.
   2. (b) A psychologist licensed under ch. 445, Stats.
   3. (c) A chiropractor licensed under ch. 446, Stats.
   4. (d) A nurse midwife certified under s. 441.15, Stats.
   5. (e) A nurse practitioner licensed under ch. 441, Stats.
   6. (f) A nurse licensed under ch. 441, Stats., who is certified as a nurse anesthetist by the American association of nurse anesthetists.
   7. (g) A dentist licensed under ch. 447, Stats.
