1. (1) The annual warrantor renewal registration form (OCI 21-051) and an annual fee of $250 shall be filed and paid at the Office of the Commissioner of Insurance on or before April 30 of each year.

2. (2) If the renewal form and fee are not filed and paid on or before April 30, a late filing fee of an $100 is due. For each consecutive 7 day period after April 30 that the form and fee are not filed or paid, an additional $100 is due.
