Subject to the review and approval of the administrator, each agency shall submit to the administrator:

1. (1) A description of the agency’s implementation and administration of a performance evaluation program which meets the requirements set forth in this chapter; and
2. (2) Identification of the use and effect of the information contained in the performance evaluation review documents.
