An application for initial certification shall be on a form provided by the department and shall be accompanied by all of the following:

1. (1) Service policies and procedures required by this chapter.
2. (2) All fees required under ss. 51.04 and 51.45 (8) (a), Stats.

3. (3) Additional information needed for certification that is requested by the department.
