Within 30 days after a participant’s date of discharge the substance use overdose treatment program shall prepare a discharge summary and enter it into the participant file. The discharge summary shall be signed and dated by a competent mental health professional and include all of the following:

1. (1) A description of the reasons for discharge.
2. (2) Any remaining participant needs at the time of discharge and the recommendations for meeting those needs.
