Company certification effective dates and expiration dates.
Effective May 1, 2009

Department of Health Services
(1)
Effective dates. Certification is effective on the date the department issues the company certificate to the applicant.
(2)
Expiration dates. Certification shall expire 24 months after the certification effective date.
