All claimants, respondents or members of formal or informal patient compensation panels shall, in the presentation and the conduct of matters before the patient compensation panels, use the following forms which have been prepared by and may be obtained from the administrative director of courts:

2. (2) Notice of Health Care Provider
3. (3) Assignment of Controversy to Panel
4. (4) Answer to Submission of Controversy
5. (5) Cross-Complaint
6. (6) Request for Hearing by Formal Panel
7. (7) Stipulation for Hearing by Formal Panel

8. (8) Petition, Consent & Order Appointing Guardian ad Litem for Settlement of Causes of Action Pursuant to s. 655.06, Stats.

   Note: Section 655.06, Stats., was repealed by 1985 Wis. Act 340.

9. (9) Stipulation for Hearing by Less than Full Panel
10. (10) Petition and Order for Consolidation
11. (11) Stipulation for Binding Findings, Order & Award
12. (12) Order by Panel to Consolidate
13. (13) Subpoena
