1. (1) As used in this section, “NDC number” means national drug code number, the universal product identifier used in the U.S. to identify a specific drug product.

2. (2) Subject to s. CSB 4.08, a dispenser shall compile dispensing data that contains all of the following information each time the dispenser dispenses a monitored prescription drug:

   1. (a) The dispenser’s full name.
   2. (b) The dispenser’s DEA registration number, if applicable.
   3. (bm) Beginning December 1, 2025, the dispenser’s NPI number.
   4. (c) The date dispensed.
   5. (d) The prescription number.
   6. (e) The NDC number of the monitored prescription drug.
   7. (f) The quantity dispensed.
   8. (g) The estimated number of days of drug therapy.
   9. (gb) The drug dosage units.
   10. (gd) The partial fill indicator.
   11. (ge) The classification code for payment type.
   12. (gm) The number of refills authorized by the prescriber.
   13. (gs) The refill number of the prescription.
   14. (h) The practitioner’s full name.
   15. (i) The practitioner’s DEA registration number, if applicable.
   16. (im) Beginning December 1, 2025, the prescriber’s NPI number.
   17. (j) The date prescribed.
   18. (L) The patient’s full name or if the patient is an animal, the animal’s name and the owner’s last name.
   19. (m) The patient’s address, or if the patient is an animal, patient’s owner’s address, including street address, city, state, and ZIP code.
   20. (n) The patient’s date of birth, or if the patient is an animal, patient’s owner’s date of birth.
   21. (o) The patient’s gender.
   22. (p) The name recorded under s. 450.11 (1b) (bm), Stats., unless exempted pursuant to s. Phar 8.06 (2).
3. (4) The board may refer a dispenser and dispenser delegate that fail to compile dispensing data as required by sub. (2) to the appropriate licensing or regulatory board for discipline.
