The licensee or their designee shall complete a report on a form provided by the department after an incident that results in death, injury, or food, waterborne or other communicable illness where an emergency medical service response is initiated by the operator. The report shall be filed with the department or its agent within 2 business days or as soon as practicable following the incident.
