A protocol established by the licensee shall be in writing and may include any of the following evaluation and treatment procedures:

1. (1) Taking a basic medical history when necessary for evaluation and treatment of an injury or illness sustained while participating in physical activity and as outlined in s. 448.956 (2) and (3), Stats. A basic medical history may include any of the following:

   1. (a) Previous medical history.
   2. (b) Previous surgical history.
   3. (c) Pertinent family medical history.
   4. (d) Current medication history including known drug allergies.
   5. (e) Relevant social history.
   6. (f) Chief medical complaint.
   7. (g) History of the present injury or illness for which the person to be treated is seeking evaluation and treatment.

2. (2) Evaluating an injury or illness sustained while participating in physical activity and as outlined in s. 448.956 (2) and (3), Stats. An evaluation may include any of the following procedures:

   1. (a) Palpation.
   2. (b) General observation.
   3. (c) Motion assessment.
   4. (d) Muscle strength tests.
   5. (e) Endurance tests.
   6. (f) Neurological assessment.
   7. (g) Joint play assessment.
   8. (h) Functional evaluation.
   9. (i) Objective physical measurement.
   10. (j) Circulatory assessment.

3. (3) Treating an injury or illness sustained while participating in physical activity and as outlined in s. 448.956 (2) and (3), Stats. Treatment may include any of the following procedures:

   1. (a) Emergency care.
   2. (b) Ultrasound.
   3. (c) Phonophoresis.
   4. (d) Electrical nerve stimulation.
   5. (e) Iontophoresis.
   6. (f) Specified diathermy.
   7. (g) Intermittent compression.
   8. (h) Traction.
   9. (i) Therapeutic massage.
   10. (j) Moist heat.
   11. (k) Paraffin baths.
   12. (L) Cryotherapy.

4. (4) Rehabilitating an injury or illness sustained while participating in physical activity and as outlined in s. 448.956 (2) and (3), Stats. Rehabilitation may include any of the following procedures:

   1. (a) Progressive resistance exercise.
   2. (b) Range of motion exercise.
   3. (c) Trigger point and manual therapy.
   4. (d) Joint mobilization for range of motion only.
   5. (e) Proprioceptive neuromuscular facilitation.
   6. (f) Functional exercise.
   7. (g) Cardiovascular exercise.
   8. (h) Aquatic exercise.
   9. (i) Taping, bracing and splinting.
   10. (j) Isokinetic exercise.
   11. (k) Isometric exercise.
   12. (L) Isotonic exercise.
5. (5) Administering specifically enumerated drugs.
