(See RCW 28C.10.050 and 28C.10.060.) In addition to the minimum licensing standards described in RCW 28C.10.050, each school must submit the following information annually for each student who participated in training during the reporting period:

1. (1) Student name, address, telephone number and Social Security number if provided by the student;
2. (2) Start date of training and date of completion or dropout;
3. (3) Enrollment status as of the end of the reporting period;
4. (4) Previous education before starting the current training program;
5. (5) Race;
6. (6) Date of birth;
7. (7) Gender;
8. (8) Disability status;
9. (9) Hispanic/non-Hispanic;
10. (10) Program title and duration (in months);
11. (11) Veteran status;
12. (12) Grade point average (GPA) or pass/fail;
13. (13) If complete, what credential earned;
14. (14) Any other information that the agency deems appropriate.
