All pages in a tariff, except the title page, must include the following:

1. (1) A page header that includes:

   1. (a) The identifying number of the tariff;
   2. (b) A page number;
   3. (c) A revision number;
   4. (d) The name of the company filing the tariff; and
   5. (e) Any applicable registered trade name.

2. (2) A page footer that includes:

   1. (a) The name of the person filing the tariff;
   2. (b) The date the page is issued;
   3. (c) The date the page becomes effective; and

   4. (d) A box that is at least three-fourths of an inch in height, spans from margin to margin and is labeled "for official use only."
